Hi Teams,
Please find your HLTA form here for your accredited medical staff to return.
Remember the minimum standard for HFNL is a Sports Medicine Australia Level one Sports Trainer.
Or a QMP ( Qualified Medical Practitioner)
Information taken from AFL National Community Football Policy Handbook:
Qualified Medical Professional (QMP) means a qualified doctor, paramedic, physiotherapist, osteopath, chiropractor, registered nurse or firefighter with Emergency Management Competency and appropriate first aid competencies.

Please remember your QMP needs Emergency Management Competency and appropriate first aid competencies.
Fire fighter is full-time. The common element is that the QMPs have training in sports injury management.
Anyone assisting on the bench needs to be listed with our league with qualifications identified on the form attached.
Also anyone assisting on the bench or field needs to be written on the team sheet and per usual. They should also be registered with play HQ and have their WWCC listed with your club (mandatory) “Child Protection(Working with Children) Act 2012 No 51.
Please send your forms to Debbie Mills and Cc (Fran) , we can then send your passes.
Thanks to those clubs who have already completed their forms.
Please understand that if we receive all info just before the start of the season we may not be able to get passes out in a timely manner.
